The Private Pay Program serves individuals fifty-five years of age or older or those with disabling conditions.

Private Pay Care Coordination is a personal service that works with you to develop a plan to assist you in these trying times, but also leaves you in control of your life.
Some common tasks performed by Private Pay Care Coordinators:
- Arranging and coordinating private pay services on your behalf
- Advocating for you or your loved one
- Providing assistance in identifying community supports
- Identifying strategies to meet your needs and desires
- Assisting with health promotion and education
- Assessing care needs if requested by customer
- Coordinating/facilitating family conferences
