
We recognize that as we age there are many new and unknown issues that present themselves. We know that searching through the maze of resources available – from entitlements, insurance, and providers of home health care and senior living options -- is daunting. We also recognize that today’s families are scattered; and help by families for ourselves or our parents is difficult.
Sara can help you through a care crisis and can help you manage current or on-going health difficulties. She will consult with you in your home, assess your personal circumstances, discuss your personal concerns, needs, and goals for your life. She will then work with you to design a plan to meet your needs. She is skilled at managing unique situations and difficult questions related to the aging network.
Some common tasks Sara might perform as a Private Pay Care Coordinator:
- Arranging and coordinating private pay services on your behalf such as home health care, household chore work, meals, home maintenance
- Advocating for you or your loved one's needs
- Providing assistance in identifying community supports/resources to assist you
- Helping you understand entitlements or benefits
- Identifying strategies to meet your needs and desires
- Assisting with health promotion and education
- Assessing care needs if requested by customer/family
- Coordinating/facilitating family conferences
- Coordinating and/or accompanying someone to the doctors’ appointments – being a second pair of eyes and ears
- Assisting in obtaining and coordinating assisted living or nursing home care
- Supporting caregivers
- Offering guidance and acting as a liaison between the person receiving assistance and their out-of-town family members and other professionals
The Private Pay Care Coordinator program provides services to persons age 55 and older or persons younger than age 55 with disabling conditions. Area IV serves the counties of Benton, Carroll, Clinton, Fountan, Montgomery, Tippecanoe, Warren and White.
NOTE: If you are unable to pay for a Private Pay Care Coordinator, but still need assistance in navigating the maze of aging resources, our Information and Referral Specialists will help you determine resources to meet your needs and advise you of the procedures to follow. (They do not coordinate care.)
