- referrals to appropriate agencies and services
- family consultations
- personal needs assessments
- Assessments and referrals for Medicaid Waiver and CHOICE in-home services and home-delivered meals.
Families caring for persons with dementia/Alzheimer’s Disease are encouraged to contact us to discuss changes due to the Disease, as well as options for care.

- The Caregiver Resource Center is a lending library of videos, books, pamphlets and resource listings related to caregiving in general and caregiving those with dementia.
- The Senior Resource Guide (PDF - Size: 96pp, 6.7mb) provides information and resources for Area IV’s 8-county service area. (FYI...this Guide is being updated in 2013.)
- SHIP counseling: Assistance in understanding Medicare issues, as well as assistance in searching for Medicare Pt. D prescription plans.
- Speakers: Will provide speakers to explain Area IV services or community resources available.
Area IV is a member of the Indiana Care Circle, an organization dedicated to working for quality care through networking with other professionals in the field.
