Area IV Agency Program Participant Rights

As a participant in Area IV Agency on Aging and Community Action Programs, Inc. services, you have the right to:

  • Receive services free from discrimination based on race, religion, color, sex, gender and gender identity, ethnicity, age, class, national origin, sexual orientation, disability, ancestry, or status as a veteran.
  • Access information about Area IV Agency by request including programs, services provided, Area IV staff, staff qualifications, and contractual relationships. If you would like more information about our agency, check out our website, Facebook, and Twitter.
  • Know your care manager and to request a change in care manager.
  • Be informed of all care management services available, even if a service is not covered, and to discuss options with your care manager.
  • Be treated with respect, dignity, consideration, and compassion.
  • Not be subject to abuse or threats of any kind.
  • Be informed about options and other services available to you and to participate in all planning for your care, including care management and in-home services if needed.
  • Be supported by Area IV Agency to collaborate on decisions with your care manager.
  • Communicate complaints to Area IV Agency and receive instructions on how to use the complaint process, including Area IV Agency’s standards of timeliness for responding to and resolving issues of quality and complaints.
  • Receive understandable information and follow-up correspondence within two (2) business days.
  • Decline participation or dis-enroll from programs and services offered by the organization.
  • Have your personally identifiable data and medical information kept confidential, to know what entities have access to your information, and know the procedures used by Area IV Agency to ensure security, privacy, and confidentiality.

Area IV Agency Program Participant Responsibilities

As a participant in Area IV Agency on Aging and Community Action Programs, Inc. services, you have the responsibility to:

  • Treat staff of this agency with respect and courtesy and not subject staff to abuse or threats of any kind.
  • Not have illicit drugs present in your home as this may jeopardize in-home services.
  • Follow the mutually agreed upon care management plan or notify your care manager if you cannot follow the plan.
  • Provide Area IV Agency with the information necessary to deliver services.
  • Inform your care manager in a timely manner if you have a change of address or phone number or concerns about your services.
  • Keep appointments to the best of your ability and promptly notify staff if you need to cancel or change the appointment time. Respond to calls and letters from Area IV to the best of your ability. Not responding timely can potentially cause a suspension of services.
  • Notify staff if you dis-enroll from the program.
